If you're living with a chronic condition, you may be able to benefit from Farm Bureau Health Plans’ Live Better program. This comprehensive program is designed to help you understand the day-to-day management of your condition, support your doctor's plan of care, and improve your quality of life. It also provides individual self-assessment and educational tools, “Care Alerts” and, when appropriate, support by phone from a nurse coach, to help you take a more active role in your own health management. Nurse coaches use evidence-based guidelines to determine what education and support may be helpful.

A Message for Providers
The Live Better at Farm Bureau Health Plans program is a resource designed to augment current practice plans and optimize care plans between office visits. An experienced group of nurses are available to help facilitate care plans and provide extra resources to your patients through a variety of means. In this program, information will be provided to your patients. Hopefully, this information will be useful in enhancing patient interactions.
Currently there are five specific chronic disease management programs including Asthma, Congestive Heart Failure, Coronary Artery Disease, COPD and Diabetes and one specially focused Maternity program.
